Grant Reporting Requirements & Upload Tool
The Foundation requires grantees to submit a grant progress report nine months after receiving the grant award. The Foundation will not make future grants to an organization unless grant evaluation forms have been received. Grantees may download a copy of the evaluation/progress report form here: Download Evaluation & Status Report Form. Please complete the Evaluation & Status Report Form then save it in a PDF file. Please hit the submit button after you have filled out the below required fields and uploaded your report below. Foundation staff will be automatically notified when your grant report is submitted.
